The commander of a Riverine squadron was sent to the bottom by the dreaded "loss of confidence" phrase. The skipper of the USS Newport News got the heave-ho following a domestic assault charge. Also the command master chief of the USS Dewey was ousted on the confidence thing.

Finally, and most recently, Brig. Gen. John Cho, head of the Army’s Western Regional Medical Command, was suspended from duty whilst an investigation of his command climate proceeds.
